Dream Team Elite Basketball Parent Agreement, Waiver & Payment Policy

Effective September 7, 2025

1. Assumption of Risk & Waiver of Liability

Participation in basketball practices, games, training, and related activities involves inherent risks, including risk of injury, accident, illness, or even death. Dream Team Elite, its owners, coaches, staff, volunteers, and affiliates are not responsible or liable for any injury, accident, illness, or death that may occur. By signing this agreement, you waive any right to bring legal action (including claims of negligence) against Dream Team Elite.

2. Medical Responsibility

I affirm my child is physically fit to participate and have disclosed any relevant medical conditions. In the event of emergency, I authorize DTE staff to obtain medical treatment as deemed necessary at my expense.

3. Payment, Auto-Billing & Refund Policy

I authorize Dream Team Elite to automatically charge my saved payment method for all program fees, monthly or seasonal, until I cancel in writing. All fees are non-refundable. No proration for missed practices or games. No bank/credit card disputes permitted. If I discontinue after billing, credits may be applied toward a future season only at DTE's discretion. Under no circumstances may a parent, guardian, or player initiate a chargeback or payment dispute with their bank, credit card company, or financial institution for any Dream Team Elite fees. All payments are final and non-refundable once processed. Participation in the program constitutes full acknowledgment of the services provided, the agreed training schedule, and the terms of this contract. Any attempt to reverse or dispute a payment outside of Dream Team Elite will be treated as a violation of this agreement. Dream Team Elite reserves the right to immediately suspend or remove the player from practices, games, or training until the balance is resolved, and may pursue legal action and collection of the owed amount, including attorney fees, processing fees, and administrative costs.

4. Price Guarantee

The DTE membership rate is a non-negotiable $195 per month, totaling $585 over the course of the full 3-month season. Players joining mid-season are still required to complete a full 3-month commitment, which may carry over into the following season if necessary. Prorated amounts may occasionally be offered at DTE’s sole discretion when joining mid-month as participation continues without interruption.

5. Fieldhouse & Jersey Fees

The $195 Fieldhouse USA league fee and $60 reversible jersey kit are required only if my child plays in league games. These are non-refundable.

6. Training Hour Guarantee & Attendance Policy

Dream Team Elite guarantees a minimum of 50 hours of training per season. Practices are not mandatory, but players must attend at least one practice per week to be eligible for weekend games. Missed sessions are the family's responsibility and do not qualify for refunds or credits.

7. Game Day Rules & Playing Time Policy

Each team must provide a parent volunteer for the scoreboard at every game or the game will not begin. Players must arrive at least 15 minutes before games and on time for practices. Parents/players must notify the coach at least 3 hours before a game if absent. Uniform rules: official DTE jersey and shorts, no jewelry, watches, or chains. Basketball shoes are required for safety. Dream Team Elite does not guarantee playing time. As a locked/private team, minutes are at the sole discretion of the coach. Concerns may be directed respectfully to Head Coach (Daniel Kovalsky).

8. Photography & Videography Release

I grant DTE permission to use photos, videos, and media of my child for promotional purposes without requiring approval.

9. Conduct & Expectations

Players and parents must uphold standards of effort, discipline, sportsmanship, and respect. Failure may result in removal from practices or games without refund.

10. Late Payment Policy

If a payment fails, I have 48 hours to update information. After 48 hours, a $25 late fee will be applied. If unpaid within 7 days, my child may be suspended until the balance is cleared.

11. Referral Program (Effective Sept 7, 2025)

The referral program applies only to new players. The referring family receives $60 Credit towards jersey fees.

12. Agreement & Legal Terms

This agreement is binding under Texas law. Any disputes shall be resolved exclusively in Denton County or Collin County courts. If any portion of this agreement is found unenforceable, the remainder shall remain in full effect. By signing, I acknowledge this is a legally binding contract required for participation in Dream Team Elite.
